You will need to complete 1, 2, and 4.  Also, for volunteers under 18 years of age form number 8 will be necessary.

Be sure to ask for your hard copy of the "Handbook for Volunteer Workers" and the "Volunteer Worker Policies and Procedures Manual" in our office.
